Texas independent primary care provider network partners with Agilon Health

Catalyst Health Network has formed a long-term partnership with Agilon Health, a value-based care platform.

The partnership will introduce Agilon’s value-based, total care model for senior patients to Catalyst’s primary care network across Texas, according to a May 2 news release. Catalyst will be Agilon’s 32nd physician group partner, spanning 14 states and 2,700 primary care physicians.

Catalyst is the largest independent primary care provider in Texas, with over 1,000 providers caring for more than 1.5 million patients across the state.
