Millennium Physician Group, a subsidiary of Mosaic Health, has partnered with physician practices in Brevard and Indian River counties in Florida, South Florida Hospital News reported Jan. 7.
Through the partnership, Millennium has added eight primary care locations to its portfolio. The practices collectively serve about 37,000 patients, according to the report.
Patients will continue to receive care at their current locations and from their existing providers. The practices will eventually be integrated into Millennium’s clinical infrastructure and value-based care resources.
